Levothyroxine is one of the most prescribed drugs in the world, and it is also one of the fussiest about what shares the stomach with it. Calcium is near the top of the list of things that get in the way.
What happens in the stomach
Levothyroxine is absorbed mainly in the jejunum and upper ileum, and it has to survive the stomach intact to get there. Calcium carbonate, the cheapest and most common form in supplements, dissolves in gastric acid and releases calcium ions that bind levothyroxine into a complex the intestine cannot take up.
The lost drug does not appear later. It passes through. This is adsorption in the gut, not an effect on metabolism, which is why the fix is entirely about timing.
Why a small loss matters more here
Most absorption interactions are tolerable because the nutrient involved has a wide margin. Levothyroxine does not. It is dosed to the microgram, titrated against blood tests over months, and small shifts in how much reaches the bloodstream translate into measurable changes in thyroid status.
Studies of the combination have found rising TSH and falling free T4 when calcium carbonate is taken alongside the morning dose, along with downstream changes in blood pressure, cholesterol, and fasting glucose. Some people on a stable dose have needed it revised upward after starting a calcium supplement, and revised back down after separating the two.
The awkward part is that both are usually taken in the morning. Levothyroxine is meant to be taken on an empty stomach shortly after waking, and calcium supplements are typically taken with breakfast to avoid stomach upset. Those two habits collide by default.
Four hours, not two
Most mineral interactions clear with a two-hour gap. This one is usually given a four-hour window, and the wider margin reflects how long calcium carbonate lingers in the gut and how little room levothyroxine has for error.
Taking levothyroxine first thing and moving calcium to the evening meal is the arrangement that fits most routines without changing either dose. Anyone splitting calcium across the day needs to look at whether the earliest of those doses still lands inside the window.
Other calcium sources count too
The interaction is not limited to a calcium tablet. Calcium-fortified orange juice, a calcium-containing antacid, and a large glass of milk all put calcium in the stomach. Antacids in particular are easy to overlook, since people rarely think of them as supplements.
