B12 absorption is the most elaborate process in human nutrition. It runs through the stomach, the small intestine, and a specific receptor in the terminal ileum, and it needs several things to go right in sequence. Proton pump inhibitors break the first step.

The step that gets removed

B12 in food does not arrive free. It is bound to protein in meat, fish, eggs, and dairy, and it has to be released before anything else can happen. That release is done by hydrochloric acid and pepsin in the stomach. Once free, B12 attaches to haptocorrin, then transfers to intrinsic factor in the duodenum, and is finally collected by receptors in the terminal ileum.

Omeprazole, esomeprazole, pantoprazole and the rest shut down the acid pumps in the gastric lining. Pepsin also needs an acid environment to function. So the first step slows, and B12 that is still bound to protein passes through without ever entering the chain.

Intrinsic factor secretion is largely intact. The receptors in the ileum are fine. The failure is upstream of all of it.

Food-bound and crystalline are different

The B12 used in supplements and in fortified foods is crystalline cyanocobalamin, and it is not attached to protein. It does not need the acid step, so it enters the pathway at haptocorrin regardless of gastric pH.

That difference is why the research on PPI-associated deficiency concerns dietary B12 specifically, and why the pattern differs from pernicious anaemia, where intrinsic factor itself is the missing piece.

It takes years

The liver stores B12 in quantities measured in years of supply, typically two to five. Someone whose absorption stops today does not become deficient this year.

That long buffer is the reason this interaction is associated with prolonged PPI use rather than short courses. Studies picking up the effect generally look at two years and beyond, and the risk climbs with duration and with dose. It also explains why the connection is so easy to miss clinically. The drug started in 2019, the tingling in the feet started in 2026, and nothing about the pair suggests cause and effect.

Bacteria and the older stomach

Two things compound it. Reduced acid allows bacterial overgrowth in the upper small intestine, and some of those bacteria consume B12 before the host gets to it. Separately, atrophic gastritis is common with age and already reduces acid output, so an older adult on a PPI has two acid-lowering influences stacked.

No timing workaround

Spacing a supplement away from the PPI dose changes nothing, because the drug suppresses acid production for most of the day and is taken continuously. This is not two substances colliding in the stomach.

Anyone on long-term acid suppression who is thinking about B12 is really asking a question about whether the drug is still needed and what their status actually is, and both of those sit with the prescriber.
